About the Role
We are seeking an experienced and compassionate Advanced Clinical Practitioner (ACP) to join our Special Allocations Scheme (SAS) service.
The SAS service provides essential primary care support for patients who have been removed from mainstream GP practices due to challenging behaviours. Many patients have complex health and social needs, including trauma backgrounds, making this a highly rewarding opportunity to deliver meaningful, person-centred care and support rehabilitation.
This role offers the chance to work autonomously while also being part of a supportive multidisciplinary team, delivering both remote triage and face-to-face consultations.
The service operates Monday to Friday between 08:00 and 18:30hrs.
Key Responsibilities
As an ACP within the SAS service, you will:
- Provide first-contact assessment, diagnosis, and management of undifferentiated physical and mental health presentations
- Deliver remote triage and face-to-face consultations using advanced clinical assessment and decision-making skills
- Manage acute presentations, long-term conditions, and preventative healthcare interventions
- Prescribe safely and appropriately within scope of practice
- Request and interpret investigations to support clinical decisions
- Deliver holistic, patient-centred care considering physical, psychological, social, and cultural needs
- Identify and respond appropriately to safeguarding concerns
- Work collaboratively with multidisciplinary teams and external healthcare providers
- Maintain accurate, confidential clinical records in line with GDPR and organisational policies
- Participate in clinical governance, audit, and quality improvement initiatives
- Support Quality and Outcomes Framework (QoF) targets while maintaining high standards of patient care
About You
Essential Requirements
- Registered with the NMC or HCPC
- Independent Prescriber qualification recorded on the NMC register
- Minimum 6 years post-qualification experience, including:
- At least 3 years in a GP primary care setting, or
- 3 years within urgent care, UTC, or Emergency Department settings
- MSc Advanced Clinical Practice pathway (or equivalent), including Non-Medical Prescribing
- Experience working within Primary Care and QoF frameworks
- Level 3 Safeguarding certification for adults and children
- Eligible to work in the UK
- Able to successfully complete an enhanced DBS check
Desirable Skills & Experience
- Teaching and mentorship qualification
- Experience within acute care and/or primary care environments
- Strong organisational and workload management skills
- Excellent communication and relationship-building abilities
- Proficiency with clinical systems such as EMIS, SystmOne, Adastra, and EPS
