We are recruiting on behalf of a busy and well-established domiciliary care and complex care provider based in Brentwood, Essex. We are looking for a dedicated and experienced Care Coordinator to join their growing team on a permanent basis.
This is an excellent opportunity for a motivated individual who is passionate about delivering high-quality care services. You will play a pivotal role in coordinating care packages, managing care staff, and ensuring that service users receive the very best standard of care. The successful candidate will thrive in a fast-paced environment and will be confident in managing complex care requirements across the local area.
As a Care Coordinator, your day-to-day responsibilities will include:
Coordinating and scheduling care visits for a large team of care workers across domiciliary and complex care packagesActing as the first point of contact for service users, families, and healthcare professionals regarding care arrangementsManaging and monitoring care rotas to ensure full coverage and continuity of care at all timesConducting care assessments, reviews, and risk assessments for service users in their own homesEnsuring all care plans are up to date, accurate, and reflective of each individual's needs and preferencesSupporting the recruitment, induction, and ongoing supervision of care staffResponding efficiently to any changes in care requirements, last-minute cancellations, or staffing issuesMaintaining accurate records and documentation in line with CQC regulations and company policiesBuilding and maintaining strong relationships with healthcare professionals, local authorities, and commissioning bodiesParticipating in the on-call rota as required, with additional payment provided for on-call dutiesEnsuring full compliance with safeguarding policies and procedures at all timesSupporting the Registered Manager with audits, inspections, and quality assurance activities---The ideal candidate will possess the following skills and qualifications:
Previous experience working as a Care Coordinator, Senior Carer, or in a similar coordinating role within domiciliary or complex careStrong knowledge of care planning, risk assessments, and CQC regulatory requirementsExcellent organisational and time management skills with the ability to manage multiple priorities simultaneouslyOutstanding communication and interpersonal skills, both written and verbalAbility to work effectively under pressure in a busy, fast-paced environmentProficiency in care management software and scheduling systemsA compassionate and person-centred approach to care deliveryStrong problem-solving skills and the ability to make confident decisionsNVQ/QCF Level 3 in Health and Social Care (or equivalent) is desirableA full UK driving licence and access to a vehicle is preferredFlexibility to participate in the on-call rota when required---**Salary:** £28,000 – £31,000 per annum, depending on experience**Working Hours:** Monday to Friday, 10:00am – 6:00pm**1 Work From Home Day** per week for improved work-life balance**On-Call payments** paid in addition to your base salaryA supportive and friendly working environmentOngoing training and professional development opportunitiesThe chance to make a real difference in people's lives every dayIf you are an enthusiastic and experienced Care Coordinator looking for your next challenge with a reputable and growing care provider in Brentwood, Essex, we would love to hear from you.
