A council in Leeds is seeking an experienced Occupational Therapist to join the Recovery Hubs service, working as part of a multidisciplinary team supporting adults to regain independence and return safely to their own homes.
The Recovery Hubs consist of Occupational Therapists working alongside Physiotherapists, Nurses, and support staff to deliver a rehabilitation-focused, 24-hour therapeutic approach aimed at promoting independence and positive outcomes for service users.
Main Duties:
- Complete holistic Occupational Therapy assessments for adults with a range of needs within a reablement/rehabilitation setting
- Develop and implement goal-led, strength-based outcome plans to support independence and recovery
- Work closely with support workers and wider MDT colleagues to ensure coordinated delivery of interventions and progress reviews
- Lead on discharge planning for service users returning to their own homes
- Apply moving and handling expertise, including risk assessment and identification of safe, person-centred solutions
- Promote single-handed care principles and reduced dependency where appropriate
- Contribute to a 24-hour therapeutic, rehabilitation-focused model of care within the Recovery Hubs
- Ensure all interventions are delivered in line with council policies, statutory requirements, and professional standards
Requirements:
- HCPC Registered Occupational Therapist
- Experience within community, reablement, or rehabilitation settings
- Strong assessment skills with a strengths-based, person-centred approach
- Confident in moving and handling risk assessment and intervention planning
- Experience working within multidisciplinary teams
- Ability to manage complex caseloads and support timely discharge planning
