About the Role
We are looking for compassionate and motivated Reablement Assistants to join a dedicated team supporting adults to regain and maintain their independence.
Working within a reablement setting, you will support individuals with their daily living activities, helping them develop the skills and confidence needed to live as independently as possible. You will work closely with service users, families, health professionals, and social care colleagues to deliver person-centred support.

Key Responsibilities
- Support individuals with personal care and daily living activities.
- Assist service users to develop and maintain independence skills.
- Follow individual support and reablement plans.
- Support service users with household tasks, meal preparation, laundry, and maintaining their living environment.
- Encourage and promote independence, wellbeing, and confidence.
- Work alongside health and social care professionals to achieve positive outcomes.
- Administer medication where required and appropriately trained.
- Participate in assessments, reviews, and support planning.
- Maintain accurate records and report any changes in service users' needs.
- Support individuals to access social, recreational, educational, and therapeutic activities.
- Promote dignity, choice, equality, and respect at all times.
About You
Essential Requirements
- Experience working within an adult social care setting.
- Experience supporting individuals to maintain or improve their independence.
- Good communication and interpersonal skills.
- Ability to build positive relationships with service users, families, and colleagues.
- Ability to follow support and reablement programmes.
- Willingness to undertake training and professional development.
- Flexibility to work shifts between 8:00am and 10:00pm on a rota basis.
- Enhanced DBS check required.
Desirable
- NVQ Level 3 in Health and Social Care or willingness to work towards it.
- Experience delivering person-centred care.
- Manual Handling certification.
- Experience working within reablement, rehabilitation, or community care services.
