Service Care Solutions are currently recruiting for an experienced Social Worker to join our client within Swansea Local Authority, working as part of a fast-paced and dedicated Home First Hospital Team.
This is a full-time position (Monday to Friday) with an initial 2–3 month ongoing contract, with a strong likelihood of extension.
The Role: You will support individuals to safely transition from hospital back into the community, promoting independence and reducing unnecessary hospital stays.
Key responsibilities include:
- Completing Care Act assessments and support planning
- Supporting safe and timely hospital discharges
- Working within a multi-disciplinary team (MDT)
- Conducting Mental Capacity Assessments and Best Interest decisions
- Liaising with families, carers, and external agencies
- Managing a complex and fast-paced caseload
- Responding to safeguarding concerns
- Must be registered with Social Care Wales
- Experience within a hospital discharge, D2A or Home First setting
- Strong knowledge of Care Act, MCA, and safeguarding legislation
- Ability to work effectively in a high-pressure environment
- £38.43 per hour Umbrella / £30.50 per hour PAYE (exclusive)
- Full-time – Monday to Friday
- Initial 2–3 month contract
- Extension likely
- Fully on-site role – no hybrid working
- £200 referral bonus for a successful candidate
