We are currently recruiting for a dedicated and compassionate Support Worker to join a respected organisation supporting young people at risk of homelessness. This is an excellent opportunity for someone passionate about helping vulnerable individuals achieve positive outcomes and greater independence.
The successful candidate will work within a supportive team, delivering person-centred support in a trauma-informed and psychologically aware environment.
Key Responsibilities
- Manage and respond to referrals, carrying out assessments and inductions for new clients
- Provide practical and emotional support to young people, including attending appointments and assisting with applications, forms, and benefits
- Support clients in accessing housing and community-based services
- Assist with delivering structured support programmes aimed at promoting independence and wellbeing
- Carry out welfare and wellbeing checks for residents
- Support clients with move-on plans and suitable accommodation options
- Maintain accurate case notes, reports, and statistical records using internal systems
Requirements
- Previous experience supporting vulnerable individuals or young people
- Experience completing support plans and risk assessments
- Strong communication and organisational skills
- A genuine passion for supporting individuals to achieve positive life outcomes
