Job Description – Support Worker (Supported Living)
- Supporting adults with learning disabilities, autism, mental health conditions, dementia, epilepsy, and complex needs within supported living settings
- Assisting with personal care, medication administration, meal preparation, and daily living activities
- Promoting independence, dignity, choice, and person-centred support
- Supporting service users to access community activities, appointments, and social engagement
- Maintaining accurate daily notes, incident reports, and care documentation
- Following safeguarding procedures and reporting concerns appropriately
- Supporting individuals with behaviours that challenge using positive behaviour support approaches
- Working in accordance with health & safety, infection control, and company policies
- Working collaboratively with healthcare professionals, families, and multidisciplinary teams
- Providing emotional support and encouraging wellbeing in a safe and supportive environment
Requirements:
- Minimum 6 months experience in Supported Living / Care settings
- Valid Right to Work in the UK
- Care Certificate including all 16 modules
- PMVA / MAPA / CPI Training Certificate
- Enhanced DBS registered on the Update Service
- Good communication and teamwork skills
- Ability to work flexible shifts including days, nights, and weekends
- Understanding of safeguarding and person-centred care principles
