Job Description – Support Worker (Supported Living)

• Supporting adults with learning disabilities, autism, mental health conditions, dementia, epilepsy, and complex needs within supported living settings
• Assisting with personal care, medication administration, meal preparation, and daily living activities
• Promoting independence, dignity, choice, and person-centred support
• Supporting service users to access community activities, appointments, and social engagement
• Maintaining accurate daily notes, incident reports, and care documentation
• Following safeguarding procedures and reporting concerns appropriately
• Supporting individuals with behaviours that challenge using positive behaviour support approaches
• Working in accordance with health & safety, infection control, and company policies
• Working collaboratively with healthcare professionals, families, and multidisciplinary teams
• Providing emotional support and encouraging wellbeing in a safe and supportive environment

Requirements:

• Minimum 6 months experience in Supported Living / Care settings
• Valid Right to Work in the UK
• Care Certificate including all 16 modules
• PMVA / MAPA / CPI Training Certificate
• Enhanced DBS registered on the Update Service
• Good communication and teamwork skills
• Ability to work flexible shifts including days, nights, and weekends
• Understanding of safeguarding and person-centred care principles
