We’re working with a well-established Health & Social Care Partnership in Falkirk to recruit an experienced Social Worker into a fast-paced Hospital Discharge Team — where you’ll play a key role in supporting safe, timely discharges and improving outcomes for vulnerable adults.

🧩 The Role

You’ll be part of a dedicated Hospital Discharge Team, working at the heart of the hospital to ensure individuals are discharged safely with the right support in place.

This is a hands-on, fast-paced role where your assessment skills and ability to coordinate care will be key.

🔍 Key Responsibilities

• Complete strengths-based assessments to support safe discharge planning
• Work closely with patients, families, and carers to develop appropriate care plans
• Assess capacity and risk, ensuring decisions align with legislation
• Collaborate with multi-disciplinary teams (nurses, doctors, AHPs)
• Arrange care packages, placements, and community support
• Ensure smooth transitions from hospital to community services
• Maintain accurate, timely case records

✅ What we’re looking for

• Qualified Social Worker
• Registered with the Scottish Social Services Council (SSSC)
• Recent experience in adult social work (hospital discharge or community ideal)
• Strong skills in assessment, care planning & risk management
• Confident working in a fast-paced MDT environment
• Excellent communication and organisational ability
