We’re recruiting a Social Worker to join the Reablement & Intermediate Care Team (RIACT) in Darlington — supporting individuals to return home safely and regain independence.

🧩 The Role

You’ll work within the RIACT team, supporting safe and timely discharges from hospital and ensuring individuals receive the right level of care post-discharge.

This is a fast-paced, rewarding role where your assessments directly impact recovery and long-term outcomes.

🔍 Key Responsibilities

• Complete strengths-based assessments to support discharge planning
• Facilitate safe and timely hospital discharges
• Arrange intermediate care and longer-term support services
• Carry out post-discharge reviews to assess ongoing needs
• Work both independently and as part of a multidisciplinary team
• Apply knowledge of the Care Act 2014
• Maintain accurate and high-quality case records

✅ What we’re looking for

• Qualified Social Worker
• Experience within Adult Social Care / Hospital Discharge / Community Teams
• Strong understanding of Care Act and discharge pathways
• Ability to work independently in a fast-paced environment
• Excellent assessment and organisational skills
