Supply Desk is delighted to be working in partnership with a welcoming and forward-thinking primary school in the Beaconsfield area to appoint a passionate and dedicated Transition Teacher. This is a fantastic opportunity to support pupils at a key point in their educational journey, helping ensure smooth and successful transitions between year groups or key stages.
This role would suit an adaptable and nurturing teacher who thrives on supporting pupils academically and emotionally, ensuring continuity in learning and strong foundations for future success.
The RoleAs a Transition Teacher, you will work across classes or year groups to support pupils during important stages of change. This may include helping children moving between EYFS and KS1, KS1 and KS2, or providing targeted support to ensure consistency in progress and attainment.
Key Responsibilities- Plan and deliver engaging, creative, and differentiated lessons to support pupils across transition phases
- Provide targeted support for pupils adapting to new curriculum expectations and classroom environments
- Work closely with class teachers to ensure continuity in learning and assessment
- Support pupils’ academic development in core subjects, including reading, writing, and maths
- Foster confidence, resilience, and independence during periods of change
- Identify and support pupils who may need additional intervention, including those with SEN
- Monitor and assess progress, using data to inform support strategies
- Build positive relationships with pupils, staff, and parents to ensure a smooth transition process
- Contribute to a positive, inclusive, and supportive school environment
- Qualified Teacher Status (QTS) – desirable
- Experience teaching in primary settings (EYFS, KS1, or KS2)
- Strong understanding of the primary curriculum and progression between key stages
- Excellent classroom management and organisational skills
- Flexible, proactive, and adaptable approach to teaching
- Strong communication and teamwork skills
- Ability to work full-time in the Beaconsfield area from September
- Experience supporting transitions between year groups or key stages
- Knowledge of both KS1 and KS2 curriculum expectations
- Experience delivering intervention or booster sessions
- Confidence using assessment data to track and support pupil progress
- Experience supporting children with SEN
