The Role

As part of a collaborative integrated Mental Health and Wellness Team, you will:

• Complete Care Act Assessments and statutory reviews.

• Undertake safeguarding enquiries and interventions.

• Provide care coordination under the CPA Framework.

• Manage a caseload of approximately 25 service users.

• Participate in the team duty rota, including screening, triaging, and managing new referrals.

• Work closely with multidisciplinary professionals to deliver high-quality, person-centred mental health services.

Requirements

• Qualified Social Worker with current Social Work England registration.

• Experience within Adult Mental Health Services.

• Strong knowledge of the Care Act, safeguarding legislation, and the CPA Framework.

• Excellent assessment, communication, and case management skills.

• Ability to work effectively within a multidisciplinary team.
