Experienced Adult Social Worker – Review Team

We're looking for an experienced Adult Social Worker to join a busy and supportive Adult Social Care Review Team.

Key Responsibilities:

• Complete strengths-based Care Act assessments, reviews and support planning.
• Manage complex cases, including safeguarding enquiries under the Care Act 2014.
• Undertake Best Interest Assessments and support Deprivation of Liberty processes.
• Work closely with health professionals and the wider Multi-Disciplinary Team (MDT) to prevent hospital admissions and promote independence.
• Participate in the team's duty function, including daily case triage and huddles.
• Chair multi-agency meetings and contribute to safeguarding, MASH, MARAC and MAPPA processes where required.
• Maintain accurate case records and work in line with Social Work England standards and the Professional Capabilities Framework (PCF).

What We're Looking For:

• Qualified Social Worker with Social Work England registration.
• Previous experience within Adult Social Care, including complex assessments and safeguarding.
• Strong knowledge of the Care Act 2014, Mental Capacity Act and Best Interest processes.
• Excellent communication skills and the ability to work effectively within a fast-paced multidisciplinary environment.
