Our client Sheffield City Council is looking for an Adults Social Worker to join their Communities Future Options service.

Job Purpose

To provide effective Social Work, Prevention, Assessment and Case Management and Reviews of Care and Support of Adults under the Care Act 2014 and other statutory requirements.

Duties and Responsibilities

  • Meet and maintain Social Work Registration requirements, maintain professional competency.
  • Through a Strengths Based Approach support people to live a fulfilling life at home, connected to the community and resources around them, and provide care and support where needed.
  • Assess and understand needs and wellbeing under the Care Act 2014 to determine eligibility, producing written assessments that evidences eligibility and evidences critical reflection and analysis to inform and provide rationale for professional decision making
  • Provide information or support to facilitate wellbeing of the people and carers of Sheffield
  • Working with other professionals and the voluntary sector to support the person and carers
  • Utilise a range of IT software systems to record actions in line with General Data Protection Regulations (GDPR)
  • Recording rationale for decisions made, co-produce support plans with the person and undertake risk assessment, applying problem-solving skills that prioritises independence, choice and recovery
  • Work within the principles of the Mental Capacity Act 2005, assessing capacity and working in a person's best interest where required
  • Leading on complex Safeguarding, VARMM, prevent harm and reduce the risks of abuse and/or neglect to adults with social care needs as guided under the South Yorkshire Safeguarding Protocols
  • Complex report writing for the purpose of legal forums. Attendance at appropriate legal forums with support.
  • Supporting, mentoring and professional supervision of social workers through the ASYE programme and CPD pathway, completing appropriate reports and processes associated with this (for example observation reports and witness statements) as agreed with the team manager/senior practitioner/professional lead.
  • Lead responsibility for supporting and mentoring student social workers and responsibility for writing relevant reports
  • Supporting, mentoring and professional supervision vocationally qualified staff in teams as agreed with the team manager/senior practitioner/professional lead
  • Research, analyse, evaluate and use current knowledge of best practice
