Advanced Care Practitioner
We are looking for experienced Advanced Care Practitioner (ACP) to join a busy, patient-focused primary care service delivering high-quality urgent and same-day care.
This is an excellent opportunity for a motivated clinician who enjoys autonomous practice while working within a supportive multidisciplinary team. You'll assess, diagnose, treat and refer patients presenting with a wide range of undifferentiated and acute conditions, helping deliver safe, effective and timely care.
Key Responsibilities
- Independently assess, diagnose and manage patients presenting with urgent and acute conditions.
- Provide same-day appointments, telephone consultations and face-to-face assessments.
- Triage patients and determine the most appropriate treatment or referral pathway.
- Request, interpret and act upon appropriate diagnostic investigations.
- Prescribe safely within your scope of practice (Independent Prescriber).
- Deliver evidence-based care in line with national clinical guidelines.
- Promote health education, disease prevention and healthy lifestyles.
- Maintain accurate clinical documentation using electronic patient record systems.
- Respond appropriately to medical emergencies within your competence.
- Work collaboratively with GPs, nurses, paramedics and wider healthcare professionals to deliver outstanding patient care.
- Participate in clinical governance, audit, quality improvement and service development initiatives.
About You
You'll be a confident and compassionate practitioner with excellent clinical judgement and strong communication skills.
Essential Requirements
- Current registration with the NMC or HCPC.
- Current Immediate Life Support (ILS) certification.
- Minor Illness and Minor Injuries qualification.
- Evidence of ongoing Continuing Professional Development (CPD).
- Experience working autonomously within urgent care, primary care, walk-in centres, minor injury units or emergency care settings.
- Excellent assessment, diagnostic and decision-making skills.
- Experience using electronic clinical systems.
- Strong understanding of NICE guidance and clinical governance.
