Band 6 - Inpatient Older Adult Discharge Social Worker

Day Webster are currently recruiting for a Band 6 Inpatient Older Adult Discharge Social Worker within Acute Mental Health Services, ensuring effective patient flow and optimal use of inpatient beds.

  • You will be required to work collaboratively with multidisciplinary teams, community services, primary care, partner agencies, and families to identify and overcome barriers to discharge from admission onwards.
  • Provide specialist knowledge of social care, housing pathways, Mental Health Act (s117), Mental Capacity Act, and No Recourse to Public Funds legislation.
  • Support carers, maintain strong links with statutory and voluntary organisations, supervise junior staff, and contribute to reducing length of stay while promoting person-centred outcomes for older adults and individuals requiring complex discharge planning.

What you need:
 Essential
  • The applicant must have a minimum of two years post graduate Social Work experience.
  • The applicant must have two years post qualification experience in a mental health setting.
  • The applicant must have experience of autonomous working as a social worker
