Job Overview
Eden Brown Synergy is currently recruiting for an experienced Adult Social Worker to join the Community Team for People with Learning Disabilities (CPTLD) at Bracknell Forest Council.
This is an excellent opportunity to work within a supportive and dynamic team, delivering high-quality social work services to adults with learning disabilities. The successful candidate will support service users, carers, and families through person-centred, strengths-based interventions, helping individuals maximise their independence, choice, and control.
This position is covering two staff members currently undertaking Social Work Apprentice Training.
Key Responsibilities
- Conduct assessments, devise care and support plans, and review and monitor plans in line with Care Act responsibilities and other statutory frameworks.
- Participate in the team's duty arrangements.
- Apply a strengths-based approach using the Conversations Model to connect people with their communities and social networks and support collaborative problem-solving.
- Work closely with individuals, advocates, and carers to develop person-centred support plans that are legal, affordable, and promote independence.
- Assess and support family carers, identifying their needs and developing appropriate support plans.
- Complete Resource Allocation System (RAS) assessments where required and present support plans for approval in accordance with the Scheme of Delegations.
- Support the arrangement and implementation of care and support plans.
- Ensure compliance with all departmental policies, procedures, and statutory requirements.
- Participate in safeguarding assessments, enquiries, and planning processes.
- Attend and contribute to Multi-Disciplinary Team meetings.
- Support individuals to remain connected to their local communities and social networks.
- Act in the best interests of individuals who lack mental capacity, applying the principles of the Mental Capacity Act 2005 to safeguard their wellbeing and rights.
Essential Requirements
- Qualified Social Worker with Social Work England registration.
- Experience working within Adult Social Care, preferably with people with learning disabilities.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation.
- Experience undertaking assessments, care planning, and reviews.
- Ability to manage a complex caseload effectively.
- Experience working within a multi-disciplinary environment.
- Excellent communication, assessment, and report-writing skills.
