About the Role
We are excited to present a fantastic opportunity for an experienced and compassionate **Care Coordinator** to join our well-established domiciliary care service based in the heart of Malvern, Worcestershire. As a Care Coordinator, you will play a pivotal role in the day-to-day coordination and scheduling of care services, ensuring that our clients receive the very best standard of care while supporting and guiding a dedicated team of care workers. Key Responsibilities
As a Care Coordinator, your responsibilities will include, but are not limited to:
Coordinating and scheduling care worker rotas to ensure full care coverage across the service at all timesActing as the first point of contact for clients, their families, and care workers regarding any queries, concerns, or changes to care plansConducting initial client assessments and regularly reviewing and updating individual care plans in line with changing needsMonitoring the quality and delivery of care services to ensure compliance with CQC regulations and company policiesSupporting the recruitment, induction, and ongoing training and development of care workersManaging and responding to any incidents, complaints, or safeguarding concerns in a timely and professional mannerBuilding and maintaining strong, trusting relationships with clients, families, healthcare professionals, and external agenciesCompleting accurate and detailed administrative tasks, including maintaining up-to-date client and staff recordsParticipating in on-call duties as required to support the smooth running of the serviceAssisting the Registered Manager in ensuring the service meets all regulatory and contractual requirements
Requirements
The ideal candidate will demonstrate the following skills, qualifications, and experience:
Previous experience working within a domiciliary care or home care setting is highly desirableStrong organisational and time management skills with the ability to prioritise a demanding workloadExcellent communication and interpersonal skills, both written and verbalProficiency in care rostering and scheduling software or a willingness to learn new systemsA good understanding of CQC regulations and compliance standards within the care sectorKnowledge of safeguarding procedures and a commitment to protecting vulnerable adultsA compassionate, empathetic, and person-centred approach to care coordinationAbility to work effectively both independently and as part of a wider teamFlexibility to participate in an on-call rota, including evenings and weekends where requiredA valid UK driving licence and access to a reliable vehicle is preferredNVQ Level 3 in Health and Social Care or equivalent qualification is advantageous
