Social Worker – Leaving Care Team

Wrexham | £38.43 per hour (Umbrella) | 6-Month Contract

Wrexham Children’s Services are looking for an experienced Social Worker to join their established and high-performing Leaving Care Team, supporting care-experienced young people aged 15–25 as they transition towards independence.

This is an excellent opportunity for a committed practitioner who is passionate about relationship-based, trauma-informed practice and helping young people achieve positive outcomes.

Key Responsibilities

• Support care-experienced young people with transition and pathway planning.
• Complete LAC reviews and statutory visits.
• Promote independence, resilience and positive outcomes.
• Apply corporate parenting principles and trauma-informed approaches.
• Build strong, trusting relationships with young people.
• Work collaboratively with families, professionals and partner agencies.
• Model best practice and provide mentoring and support to colleagues.
• Maintain accurate case records and ensure statutory requirements are met.
• Apply relevant Welsh legislation, guidance and social care frameworks.

Essential Requirements

• Recognised Social Work qualification – essential.
• Experience working with children, young people or care-experienced young people.
• Experience completing LAC reviews and statutory visits.
• Good understanding of Leaving Care and transition services.
• Knowledge of relevant Welsh legislation and children’s social care requirements.
• Strong relationship-based and trauma-informed practice skills.
• Ability to work effectively within a supportive, high-performing team.

Location: Wrexham
Rate: £38.43 per hour (Umbrella)
Contract: Temporary – 6 months
