CIC Placement Tracker Officer

Full time – Hybrid
Role Responsibilities:
- Review cohorts of children and young people within high-cost residential placements.
- Analyse care plans, case records and placement histories to identify opportunities for improved outcomes and permanency planning.
- Work alongside operational social workers and managers to challenge and support placement decision making.
- Identify and progress appropriate transitions from residential care to fostering arrangements where this is in the child's best interests.
- Track actions, outcomes and progress against agreed care plans.
- Produce detailed reports, recommendations and case reviews.
- Utilise data and management information to identify trends, opportunities and risks.
- Support the delivery of the service's cost improvement programme by ensuring placement decisions align to children's assessed needs and outcomes.
- Provide professional challenge and constructive scrutiny of recommendations and decision-making processes.
- Work independently while maintaining strong relationships with operational teams and stakeholders.
- Contribute to the development of emerging processes and frameworks relating to placement review activity.
- Liaise with fostering providers, including Independent Fostering Agencies (IFAs), and other relevant partners
