Community Care Officer

We are currently recruiting for a Community Care Officer to support a public client in Nottingham.
You will work as part of a professional team supporting vulnerable adults and their families, completing assessments, developing care plans and coordinating services to meet assessed needs.
Key responsibilities:
* Undertake adult and carer assessments.
* Develop and review person-centred care plans.
* Manage allocated caseloads, including risk management.
* Coordinate services and ongoing care provision.
* Maintain accurate case records and reports.
* Work closely with Social Workers, professionals and partner agencies.
* Participate in multidisciplinary meetings and duty rotas.
* Promote safeguarding, confidentiality and best practice.
Key requirements:
* Experience working within Adult Social Care.
* Strong assessment and case management skills.
* Knowledge of safeguarding procedures and relevant legislation.
* Excellent communication and report-writing skills.
* Ability to work independently in a fast-paced environment.
* Experience working collaboratively with professionals and partner agencies.
Contract Length: 3-6 months with expectation of an extension
Pay Rate: £22.52 per hour
Working pattern: 37 hours per week
