• Pay Rate: £17 - £20 per hour

We are looking for caring, motivated, and compassionate Band 2/3 Healthcare Support Workers (HCA) to join our community teams across the East of England.

This role offers the opportunity to support patients in their own homes and community settings, helping them maintain their independence, improve their wellbeing, and receive high-quality care close to home.

What You'll Do

• Provide personal care and support to patients in the community

• Assist with activities of daily living and promote independence

• Monitor and report changes in patients' conditions

• Support nurses and multidisciplinary teams in delivering care plans

• Work closely with patients, families, and healthcare professionals

What We're Looking For

Essential

• Minimum 6 months' NHS or Community Healthcare Assistant experience

• Full UK Driving Licence and access to a vehicle

• Care Certificate

• Experience working within community nursing teams

• Knowledge of basic observations and patient monitoring

• Experience using electronic patient records

You will play a key role in helping individuals maintain their independence, improve their wellbeing, and access the support they need within their own homes and communities.

What You'll Do

• Support service users with their health, wellbeing, and social care needs

• Carry out assessments and contribute to person-centred care plans

• Work closely with healthcare professionals, families, and partner agencies

• Promote independence and positive health outcomes

• Keep accurate and timely records

• Follow safeguarding, confidentiality, and professional standards

• Participate in team meetings, supervision, and training

What We're Looking For

Essential

Minimum 6 months of  NHS or community healthcare experience

Experience working with vulnerable adults, families, children, or community services

Good organisational and time management skills

Understanding of safeguarding and confidentiality

Full UK driving licence and access to a vehicle
