From £27,000 plus Bonus and On-Call Allowance

Full Time - Monday–Friday + Shared On-Call

Apollo Home Healthcare is seeking an experienced Complex Care Coordinator to support the delivery of high-quality, person-centred care across Doncaster and surrounding areas.

You will coordinate care packages, manage staff rotas, schedule Healthcare Assistants and Nurses, and act as the main point of contact for clients, families, and healthcare professionals. You will also support multi-agency working and help ensure care is delivered to the highest standards.

Key Responsibilities

  • Manage rotas and staff scheduling
  • Match care staff to client care packages
  • Support clients, families, and healthcare professionals
  • Monitor care delivery and care plan compliance
  • Attend multi-agency and ICB meetings
  • Participate in the shared on-call service (non-clinical)

Requirements

  • Previous Care Coordinator experience
  • Strong organisational and scheduling skills
  • Excellent communication and relationship-building abilities
  • Understanding of person-centred care and care planning
  • Calm, solution-focused approach to decision-making
  • Full UK Driving Licence and access to a vehicle
