We’re looking for a Health and Wellbeing Practitioner to join our Substance Misuse Service at HMP Brixton on a full-time basis, working 35 hours per week.
This is an opportunity to work at the heart of an integrated healthcare team, supporting men in custody to address drug and alcohol use, improve their wellbeing and take meaningful steps towards recovery and resettlement.
Based on-site within a busy Category C resettlement prison, you’ll work as part of a multidisciplinary team delivering recovery-focused, trauma-informed psychosocial interventions to adults aged 18+ with substance misuse needs. From assessment and care planning to structured 1:1s, group work and release planning, this is a varied and rewarding role with real impact.
What you’ll be doing
In this role, you’ll manage a caseload and deliver high-quality health and wellbeing support to service users throughout their time in custody. You’ll work closely with healthcare and partner agencies to ensure individuals receive the right support at the right time, from arrival into prison through to release.
Key responsibilities include:
- Managing a caseload of service users with drug and/or alcohol support needs
- Carrying out risk, initial and comprehensive assessments
- Delivering 1:1 interventions, group sessions and structured treatment programmes
- Developing care plans, treatment plans and release plans
- Providing harm reduction advice, guidance and recovery-focused support
- Working closely with healthcare, probation, housing, employment and external agencies to support through-care and resettlement
- Maintaining accurate case records, care documentation and data capture in line with required standards
- Supporting service targets, KPIs, audits and continuous service improvement
- Contributing to a safe, integrated and multidisciplinary working environment
What we’re looking for

- Experience of working with people affected by substance misuse and/or complex needs
- The ability to manage a caseload and deliver structured, recovery-focused support
- Experience of carrying out assessments, care planning and risk management
- Confidence delivering 1:1 interventions and group work
- Strong partnership-working skills and the ability to collaborate with healthcare and external agencies
- A trauma-informed, person-centred approach to supporting service users
- Good organisational skills, with the ability to maintain accurate records and meet deadlines
- Resilience, professionalism and confidence working within a prison environment

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
