Locum Out of Hours ACP / Paramedic Practitioner / Prescribing Pharmacist   £40–£55 per hour | West Midlands | Saturday shifts

If you enjoy autonomous practice and want occasional out-of-hours work where your clinical judgement is genuinely relied upon, this is a good opportunity to support patients across community hospital settings.
 

The service provides out-of-hours clinical cover for adult inpatients receiving rehabilitation, intermediate care, sub-acute, palliative and step-up care. You will support nursing teams with assessment, treatment decisions, prescribing where within scope, escalation planning and advice.
 

It is a varied role rather than a traditional clinic list. Presentations can include acute deterioration, pain, falls, frailty and sub-acute illness, with a mixture of telephone advice, remote review and face-to-face assessment depending on clinical need.

  For independent prescribers, the role includes medication review, medicines optimisation, electronic prescribing and treatment adjustment using clinical and blood results. Non-prescribing practitioners can also be considered where the service model allows, with prescribing recommendations escalated appropriately.
 

Who they are looking for. The service can consider experienced:

  • Advanced Clinical Practitioners.
  • Paramedic Practitioners.
  • Prescribing Pharmacists.
  You will need current professional registration, strong advanced assessment and clinical decision-making skills, and experience within urgent care, primary care, community, frailty, emergency or intermediate care. You must also be comfortable assessing acutely unwell adults and working autonomously within your scope of practice.
