The role

You will support working-age adults to achieve their goals and live as independently as possible. Working within a collaborative, strengths-based service, you will assess needs, develop person-centred support plans and coordinate the right resources to deliver positive outcomes.

What you’ll need
 
  • A recognised Social Work qualification
  • Current Social Work England registration
  • Experience working with adults, ideally within a strengths-based practice model
  • Strong assessment, communication and partnership-working skills
  • A commitment to inclusive, person-centred practice
