If you're an experienced Registered Nurse with a background in complex care, this is an opportunity to step away from shift work and into a hybrid Monday-Friday role where you'll have genuine autonomy, build lasting relationships with clients and families, and make a real difference every day.
I'm recruiting on behalf of one of the UK's leading complex care providers, supporting adults with highly complex clinical needs to live safely and independently in their own homes across West Yorkshire.
As a Nurse Case Manager, you'll take ownership of a varied caseload, working closely with clients, families and multidisciplinary teams to ensure every individual receives exceptional, person-centred care.
The Role
You'll oversee complex care packages from initial assessment through to ongoing clinical management, ensuring safe, high-quality care is consistently delivered within the community.
Your responsibilities will include:
- Completing holistic client assessments and developing personalised care plans
- Managing and reviewing complex care packages across the community
- Providing clinical leadership, supervision and support to care teams
- Working closely with clients, families and multidisciplinary professionals to coordinate care
- Monitoring clinical governance, quality and compliance
- Identifying opportunities to continually improve care and outcomes
You'll be a confident, compassionate nurse who enjoys working autonomously while remaining part of a supportive clinical team.
To be considered, you'll have:
- Registered General Nurse (RGN) qualification
- Active NMC registration
- Recent hands-on experience managing tracheostomy and ventilator care (within the last two years)
- Experience supporting, mentoring or supervising care staff
- Strong clinical assessment and care planning skills
- Full UK driving licence
- Salary up to £45,000 DOE
- Monday-Friday working pattern (9am-5pm)
- No nights, weekends or bank holidays
- £0.45 per mile mileage reimbursement
