If you're an experienced Registered Nurse with a background in complex care, this is an opportunity to join a leading national provider in a Monday-Friday role offering autonomy, flexibility and the chance to make a genuine difference to people living in their own homes.

I'm recruiting for a Nurse Case Manager to manage adult complex care packages across Merseyside and West Lancashire, supporting clients with high-acuity clinical needs to live safely and independently within the community.

The role offers a varied hybrid working pattern, with approximately 2 days per week office-based and 3 days spent visiting clients or working from home.

The Role

You'll take ownership of a caseload of complex care packages, overseeing each client's care from initial assessment through to ongoing clinical review and management.

Key responsibilities will include:
  • Completing holistic clinical assessments and developing personalised care plans
  • Managing and reviewing complex care packages within the community
  • Providing clinical leadership, supervision and support to care teams
  • Working closely with clients, families and multidisciplinary professionals
  • Ensuring high standards of clinical governance, quality and compliance
  • Identifying and implementing improvements to care delivery and outcomes
About You

To be considered, you'll need:
  • Registered General Nurse (RGN) qualification
  • Active NMC registration
  • Recent hands-on experience with tracheostomy and ventilator care
  • Experience supporting, mentoring or supervising care staff
  • Strong clinical assessment and care planning skills
  • Confidence working autonomously within the community
  • Full UK driving licence
