Outreach Support Officer

Good Afternoon,

I am currently representing Enfield Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience

We are looking for a Outreach Support Officer this role will be based at: EN1 3ES

The right candidate will:

To provide care and support to adults living with dementia, learning disabilities, autistic spectrum disorder, older people and people with a sensory or physical impairment, either in the community or home based. The focus being to support individuals to regain confidence, maximise independence and to continue living at home and access the community. The service provides Long- or short-term support

• As required within the service, work with service users to assess, design, plan, implement and support care plans/management of cases as directed by the line manager to maximise individual service user opportunity and development.
• Assist with establishing, developing and sustaining effective working relationships with service users.
• Provide appropriate and practical assistance and support to meet the needs and requirements of service users, individually and as a team.
• As agreed within the service to undertake the role, duties and responsibilities of a key/case worker.
• To assist the Manager/Team Leader in ensuring that service users have appropriate access to advocacy and where appropriate to the service, are empowered to self advocate.
• Actively support and promote the inclusion and participation of service users using a range of interventions e.g. group work, user participation.

We require the following:

• A Social Care qualification i.e.  NVQ/QCF 3 in Social Care or equivalent.
• Minimum of 2 years’ Experience of supporting vulnerable adults with home and community based personal care and support.
• An understanding of domiciliary care services regulatory requirements
