Are you an experienced Registered Nurse with a passion for specialist palliative and end-of-life care?

An exciting opportunity has arisen for an experienced Senior Nurse to join a specialist 24-hour palliative care helpline and care coordination service supporting patients, families, carers and healthcare professionals across North West London.
The Role
As a Senior Nurse within the service, you will play a key role within an Integrated Care Coordination Hub, providing specialist palliative care advice, clinical triage and support.
The service operates 24 hours a day, meaning you will be required to work a variety of shifts, including night shifts.
Your responsibilities will include:
  • Providing expert telephone advice and support to patients, families, carers and health and social care professionals.
  • Undertaking holistic assessments and clinical triage to ensure patients receive the most appropriate care and support.
  • Supporting patients to remain at home wherever possible and helping to avoid unnecessary hospital admissions.
  • Working collaboratively with community, hospice, acute and social care teams to coordinate patient care.
  • Providing specialist palliative and end-of-life care advice to healthcare professionals across a range of settings.
  • Recognising clinical deterioration, safeguarding concerns and escalating needs, taking appropriate action where required.
  • Maintaining accurate and timely electronic patient records.
  • Contributing to high standards of clinical governance and professional practice.
  • Acting as a senior clinical resource within the service and promoting excellent standards of patient-centred care.
About You
We are looking for a motivated and experienced clinician who can demonstrate:
  • Registration as a Registered Nurse (NMC) or Registered Paramedic (HCPC).
  • Current professional registration.
  • Significant post-registration clinical experience.
  • Experience caring for patients with palliative, end-of-life or complex long-term conditions.
  • Excellent communication, assessment and clinical decision-making skills.
  • The ability to provide calm reassurance and compassionate support in sensitive and emotionally challenging situations.
  • A strong commitment to patient-centred care and collaborative working.
Experience in telephone triage, community nursing, hospice care, urgent care or care coordination would be advantageous.
