About the Role
Southampton City Council is seeking an experienced Personal Advisor to join its established Leaving Care service on a short-term contract.
This is a vital frontline role supporting care leavers aged 18–25, helping them successfully transition into independent adulthood. You will play a key role in improving life chances by providing practical, emotional, and aspirational support tailored to individual needs.
You will hold a caseload of young people and work closely with them, their networks, and partner agencies to ensure consistent and effective support.
Key Responsibilities
- Case-hold a cohort of care leavers aged 18–25
- Develop, review and implement Pathway Plans
- Support young people with housing, education, employment and training (ETE)
- Promote independence through life skills development
- Work collaboratively with multi-agency professionals and support networks
- Participate in a duty rota to respond to urgent or emergency needs
- Provide consistent, relationship-based support to vulnerable young adults
- Ensure accurate recording and compliance with statutory requirements
The successful candidate will have:
- Experience working with care leavers, looked after children or young people
- Strong background in case management or key working roles
- Knowledge of leaving care legislation and Pathway Planning
- Ability to build trusting, supportive relationships with young people
- Experience working within a multi-agency environment
- Strong communication, organisation and time management skills
- Ability to work independently and manage a varied caseload
- A commitment to supporting young people into independence
- Hybrid working with expectation of office attendance (minimum 2 days per week)
- Travel across the region required to meet young people
