An exciting opportunity has become available for an experienced and motivated Reablement Officer to join a specialist community day care and reablement centre in Bournemouth.
This unique service provides short-term rehabilitation, reablement and end-of-life care, supporting up to 35 clients to regain confidence, improve independence and achieve their personal goals. Located within a welcoming community setting, the centre provides a modern environment where clients receive tailored support from a dedicated multi-disciplinary team, including healthcare and therapy professionals.
The service focuses on improving lives and building independence, supporting individuals following illness, injury or hospital discharge, with the aim of helping people maintain their skills, improve wellbeing and remain as independent as possible.
As a Reablement Officer, you will provide leadership and support to a team of Reablement Assistants, ensuring high-quality, outcome-focused care is delivered. You will oversee staff performance, coordinate workloads, support training and development, and work closely with clients, families and professionals to ensure agreed goals are achieved.

Responsibilities include:
- Managing and supporting a designated team of Reablement Assistants
- Allocating workloads and planning staff schedules
- Providing supervision, support and guidance to care staff
- Supporting recruitment, appraisals and identifying training needs
- Ensuring staff deliver high-quality, outcome-based reablement support
- Completing first visits and review visits with clients
- Undertaking risk assessments and providing advice to clients, families and professionals
- Supporting individuals to achieve agreed goals and maintain independence
- Liaising with healthcare professionals and other services involved in client care
- Ensuring compliance with policies, procedures, CQC Fundamental Standards and the Care Act 2014
- Maintaining accurate electronic records and case documentation
- Supporting safe working practices and ensuring appropriate equipment and PPE is available
What we're looking for:
- NVQ Level 3 in Health & Social Care (or equivalent) – Essential
- Previous experience within health, social care, rehabilitation or reablement services
- Experience working in a Supervisor, Team Leader or Senior Care role
- Experience managing, supporting or mentoring care staff
- Full UK driving licence with access to a vehicle
- Business use motor insurance is essential
- Strong leadership, communication and organisational skills
- Ability to work independently and make decisions in a fast-paced environment
- A caring, professional and proactive approach
- Passion for promoting independence and delivering high-quality person-centred support

This is a community-focused role requiring flexibility to work a rota covering early and late shifts across weekdays and weekends. The successful candidate will support clients with a range of needs and help ensure they receive the right support to maximise their independence and quality of life.
