Job Purpose

To provide safe, effective, and compassionate clinical nursing care to individuals with complex disabilities and health needs, ensuring the highest standards of physical and emotional wellbeing. The RGN will work as part of a multidisciplinary team to assess, plan, implement, and evaluate individualised care plans, while promoting the dignity, choice, and independence of the people we support.

Key Responsibilities

  • Deliver skilled nursing care to individuals with a range of complex and long-term conditions, including physical disabilities, neurological conditions, and associated health needs.
  • Carry out clinical procedures such as PEG feeding, tracheostomy care, catheter care, wound management, and medication administration in line with current NMC guidelines.
  • Undertake health assessments, monitor vital signs, and respond promptly to changes in condition, escalating concerns appropriately.
  • Develop, review, and update individualised care and support plans in collaboration with the individual, their family, and the wider care team.
  • Administer and manage medications safely, including controlled drugs, in accordance with organisational policy and NMC standards.

Person-Centred Support

  • Promote choice, dignity, and independence in all aspects of care delivery.
  • Support individuals to access healthcare appointments, therapies, and community activities as needed.
  • Build positive, trusting relationships with the people supported, their families, and advocates.

Team & Leadership

  • Provide clinical guidance, supervision, and support to support workers and healthcare assistants.
  • Act as a positive role model, promoting best practice in nursing and care delivery.
  • Participate in multidisciplinary team meetings, case reviews, and care planning discussions.
  • Support the induction, training, and competency assessment of care staff in clinical tasks delegated under nursing supervision.

Health & Safety / Compliance

  • Maintain accurate, timely, and legible clinical records in line with NMC and organisational standards.
  • Ensure compliance with infection control, safeguarding, and health and safety policies.
  • Participate in audits, quality assurance activities, and continuous improvement initiatives.
  • Report and document incidents, accidents, and safeguarding concerns promptly and appropriately.

Professional Development

  • Maintain up-to-date NMC registration and revalidation requirements.
  • Engage in regular supervision, appraisal, and continuing professional development.
  • Keep skills and knowledge current through training relevant to complex disability nursing.

Essential Criteria

  • Registered General Nurse (RGN) with current, valid NMC registration.
  • Demonstrable post-registration experience, ideally within disability, complex care, learning disability, or community nursing settings.
  • Sound knowledge of clinical procedures relevant to complex health needs (e.g. tracheostomy care, enteral feeding, catheter care, epilepsy management).
  • Strong understanding of safeguarding principles and mental capacity legislation.
  • Excellent communication skills, with the ability to build rapport with individuals who may have communication difficulties.
  • Ability to work both independently and collaboratively within a multidisciplinary team.
  • A genuine commitment to person-centred, rights-based care.

Desirable Criteria

  • Experience in medication management systems and care planning software.
  • Mentorship or supervisory experience.
  • Additional clinical training (e.g. epilepsy/rescue medication administration, PEG management, PBS awareness).
  • Enhanced DBS check on update service required or willingness to obtain one through us.
