Salary: £25,877–£29,000 per annum (depending on experience)

Our client is seeking a dedicated and compassionate Rehabilitation Support Worker to join their successful CFO Evolution team. This rewarding role involves supporting individuals in custody to develop the confidence, skills and resilience needed to successfully reintegrate into their communities.
Working within a prison environment, you'll play a key role in helping participants overcome barriers, engage with support services, and prepare for life after release.
  
The Role
As a Rehabilitation Support Worker, you will manage a caseload of participants throughout their custodial journey, from initial assessment through to release planning and resettlement.
You'll provide tailored support through one-to-one sessions and group interventions, helping individuals identify their goals, overcome barriers, and access the services they need to achieve sustainable outcomes.
Working collaboratively with colleagues, peer mentors and external partner organisations, you'll help participants build the foundations for a successful transition back into the community.
  
Key Responsibilities
  • Manage a caseload of participants, providing consistent support throughout custody.
  • Complete assessments and develop personalised action plans.
  • Deliver engaging one-to-one support and group-based interventions.
  • Encourage motivation, engagement and positive behavioural change.
  • Support participants to access housing, wellbeing services, training, education, substance misuse support and community resources.
  • Work collaboratively with peer mentors, volunteers and partner agencies to achieve positive outcomes.
  • Maintain accurate records using case management systems.
  • Follow safeguarding procedures and manage risk appropriately.
  • Work towards individual and team performance targets.
  • Contribute to continuous improvement by sharing knowledge and best practice with colleagues.
About You

You may have experience in:
  • Probation services
  • Social care
  • Prison or custodial settings
  • Rehabilitation or resettlement services
  • Substance misuse support
  • Mental health services
  • Housing or community outreach
  • Coaching, mentoring or employability support

Skills and Experience
Essential:
  • Excellent communication and interpersonal skills.
  • Strong relationship-building and engagement skills.
  • Good organisational and time management abilities.
  • Ability to work independently and as part of a team.
  • Experience using IT systems and maintaining accurate records.
  • Comfortable working in a target-driven environment.
  • A professional approach to safeguarding and risk management.
Desirable:
  • Level 3 Information, Advice and Guidance (IAG) qualification.
  • Experience managing a caseload.
  • Previous experience within probation, criminal justice, social care, rehabilitation or related support services.
