Senior Branch Nurse Complex Care

Our client is a leading provider of complex care, offering support to people living in their own homes in the community, some independently and some with their family.

They are looking to recruit a Senior Branch Nurse to manage the clinical aspects of the service across primarily East and West Sussex. Reporting to the Operational Manager the successful applicant will be responsible for:

• Overseeing case management for complex care clients, including auditing care plans and risk assessments and reviews.
• Ensuring that clinical policies, procedure and forms are fit for purpose
• Support and supervision of Nurse Supervisors
• Preparing reports for and participating in the monthly Clinical Governance meetings
• Participating in discharge meetings, care reviews and liaison with health professionals
• Participating in on-call escalation rota.
• Seeking new and appropriate services
• Networking with professional colleagues to promote best practice and service standards.

Key criteria for the role will include:

• An active PIN
• Understanding of community- based nursing / case management
• Understanding of CQC requirements for TDDI registrations
• Strong clinical skills
• Disciplined with a strong work ethos
• Team player, with good leadership skills
• Full, current driving licence and use of own car
• Must be based in East Sussex and be prepared to travel extensively in the area
