We are seeking a qualified and experienced Adult Social Worker to support adults with care and support needs, helping individuals maintain their independence, wellbeing and quality of life.
The successful candidate will work collaboratively with adults, families, carers and partner agencies to assess needs, manage risk and develop appropriate care and support plans.
Key Responsibilities-
Undertake Care Act assessments and reviews of adults with care and support needs.

Develop person-centred care and support plans focused on strengths, independence and positive outcomes.

Manage an allocated caseload, prioritising complex and safeguarding cases appropriately.

Assess and manage risk, including responding to safeguarding concerns.

Complete Mental Capacity Assessments and contribute to best-interest decision-making where required.

Work collaboratively with health, social care and other multi-disciplinary professionals.

Coordinate appropriate care packages, support and community services.

Promote reablement and independence, wherever appropriate.

Advocate for adults and ensure their wishes, views and desired outcomes remain central to planning.

Maintain accurate and timely case records in accordance with statutory and organisational requirements.

Participate in supervision, meetings, reviews and professional development.

Apply relevant legislation, policies, procedures and professional standards to all areas of practice.
The successful candidate will have:

A recognised Social Work qualification.

Current Social Work England registration.

Relevant experience within Adult Social Care.

Strong knowledge of the Care Act 2014 and adult safeguarding.

Experience undertaking assessments, care planning and reviews.

Experience of risk assessment and safeguarding.

Understanding of Mental Capacity Act principles and best-interest decision-making.

Strong communication, analytical and report-writing skills.

Experience working effectively within multi-disciplinary teams.

A person-centred, strengths-based approach to social work.

The ability to manage a caseload independently and make sound professional decisions.
