Social Worker - Multi - disciplinary Team - Brent

Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the SMART Multi-Disciplinary Team within the London Borough of Brent.

The SMART service provides practical and psycho-social support for adults who have complex and intersecting needs. The service works with individuals who may experience a cycle of homelessness, mental health admissions, drug and alcohol dependencies, and contact with criminal justice and probation services.

The service supports a particularly vulnerable group who are often multiply excluded from traditional services because of the complexity of their needs and the difficulties they may experience engaging with conventional service models.

About the Service

• The SMART service is designed to bridge gaps between different agencies and services by providing intensive, flexible and coordinated support.
• The team works with individuals who may have a very low quality of life and significant safeguarding risks, including risks relating to neglect, abuse and serious harm.
• Due to the complexity of their presenting needs, individuals can often be passed between different agencies where their primary need is unclear or they do not meet the eligibility criteria for traditional statutory services.
• The SMART team provides a flexible approach to engagement and assessment, recognising that trauma and multiple exclusions can make it difficult for individuals to advocate for themselves or engage through traditional office-based services.

Key Responsibilities

• Provide practical and psycho-social support to adults with complex and intersecting needs.
• Undertake assessments of housing, daily living, health and social care needs.
• Build effective relationships with individuals who may have experienced multiple exclusions from services.
• Use assertive outreach approaches to engage individuals who may struggle to access traditional services.
• Apply trauma-informed principles when working with individuals who have experienced significant trauma and disadvantage.
• Use harm reduction approaches to support individuals experiencing substance misuse and other complex needs.
• Identify, assess and respond appropriately to safeguarding risks.
• Work collaboratively with housing, health, social care, mental health, criminal justice, probation and other partner agencies.
• Contribute to effective multi-agency management and coordinated support planning.
• Support individuals to access appropriate longer-term services and achieve improved wellbeing and quality of life.
• Work flexibly with individuals whose needs may not fit neatly within traditional service pathways.

Pay Rate

• £32 p/h Umb
