The Oakleaf group are seeking to recruit Physical Health Assistant to join the team at their site in Hartwell, Northampton.
To support the delivery of high-quality physical healthcare within the Neuro Rehabilitation Service by assisting with health promotion, routine monitoring, administrative coordination and ensuring service users receive timely access to healthcare services. The post holder will work as part of the multidisciplinary team to promote positive health outcomes and maintain accurate records whilst supporting individuals with complex neurological needs.
About the service - The Oakleaf Group
This is an exciting opportunity to join an established multi disciplinary team working within the field of acquired brain injury. The Oakleaf Group currently caters for a male client base specialising in neurological rehabilitation across various sites.
This role is based in one of 4 units at our Hilltop House site at Hartwell in Northampton, currently rated as Outstanding by CQC.
Working for this Leading Care Provider you will be instrumental in the provision of care and therefore it is desirable that you have a proven track record working in a similar position within the care sector.
The Role
You will:
Physical Health Monitoring
- Carry out routine monthly weight monitoring and accurately record results.
- Support routine physical health observations as delegated.
- Update and maintain wound monitoring records under the direction of registered clinicians.
Support intervention audits including:
- Repositioning
- Food and fluid charts
- PEG care documentation
- Input/output monitoring
- Physical health observations
- Escalate concerns to qualified staff in accordance with service procedures
Health Appointments
- Arrange and coordinate appointments with:
- GP surgeries
- Hospital outpatient services
- Other healthcare professionals as required.
- Support individuals to attend appointments where appropriate.
- Ensure outcomes from appointments are communicated to the clinical team and recorded appropriately.
Physical Health Administration
- Order and maintain physical healthcare stock and clinical supplies.
- Order continence products in line with individual care plans.
- Maintain accurate records of stock levels and report shortages promptly.
- Support ward administration including archiving records and onboarding documentation.
Health Promotion
- Support delivery of health awareness campaigns throughout the year.
- Promote healthy lifestyles including:
- Smoking cessation
- Infection prevention
- Assist with implementation of health promotion initiatives across the service.
Clinical Support
- Provide feedback following appointments to ensure continuity of care.
- Support delivery of ear care programmes where appropriately trained.
- Assist with immunisation programmes where appropriate and within competency.
- Undertake venepuncture if trained, competent and authorised.
- Support units to monitor emergency bag, anaphylaxis and first aid stock checks
Champion Roles
- Support the implementation and promotion of quality improvement initiatives including:
- Dementia Champion
- Hydration Champion
- Dignity Champion
- Vision Champion
- Other health-related champion roles as identified by the service.
Record Keeping
- Maintain accurate, timely and confidential records.
- Ensure all documentation complies with organisational policies, GDPR and professional standards.
