Dedicated and compassionate Support Worker responsible for providing high-quality care and support to individuals with physical disabilities, learning disabilities, mental health conditions, autism, or other support needs. Committed to promoting independence, dignity, wellbeing, and social inclusion while ensuring a safe and supportive environment.

Key Responsibilities

  • Deliver person-centred care and support in accordance with individual care plans.
  • Assist service users with daily living activities, including personal care, dressing, bathing, meal preparation, and household tasks.
  • Support individuals in developing life skills and maintaining independence.
  • Accompany clients to appointments, educational activities, community events, and social engagements.
  • Administer medication and maintain accurate medication records in line with organisational policies.
  • Monitor and report changes in clients' physical, emotional, and mental wellbeing.
  • Maintain accurate documentation, progress notes, and incident reports.
  • Promote safeguarding, equality, diversity, and inclusion at all times.
  • Provide emotional support and encouragement to service users and their families.
  • Work collaboratively with healthcare professionals, social workers, and multidisciplinary teams.
  • Ensure compliance with health and safety regulations, confidentiality requirements, and organisational procedures.
  • Respond effectively to emergencies and challenging situations while maintaining a calm and professional approach.

Qualifications and Experience

  • Previous experience in a care, support, or healthcare setting preferred.
  • NVQ/QCF Level 2 or Level 3 in Health and Social Care (desirable).
  • Understanding of safeguarding vulnerable adults and children.
  • Enhanced DBS check (where applicable).
  • Basic IT and administrative skills.
