About the Role

We are currently seeking a dedicated and experienced Team Leader / Rehabilitation Support Worker to support an 18-year-old male client living in the CR3 area.

This is a highly rewarding opportunity to work within a dynamic multidisciplinary team (MDT), supporting a young individual with complex needs through a structured rehabilitation pathway.

The role focuses on neurorehabilitation and spinal injury support, bridging the transition from hospital to home. You will play a key role in delivering tailored care plans, promoting recovery, and enhancing the clientโ€™s independence and quality of life.

Key Responsibilities

  • Deliver high-quality, person-centred care in the clientโ€™s home
  • Support rehabilitation programmes and therapy goals
  • Assist with manual handling and mobility support
  • Administer and monitor medication safely
  • Work collaboratively with healthcare professionals and therapists
  • Monitor progress and report changes in condition
  • Provide emotional and psychological support to the client
  • Help prevent complications and promote long-term wellbeing

Person Specification

Essential Skills & Experience

  • Experience supporting individuals with spinal cord injuries and complex needs
  • Strong manual handling skills
  • Experience in medication administration and monitoring (hospital/ICU background desirable)
  • Understanding of rehabilitation and recovery pathways
  • Excellent communication and interpersonal skills
  • Good IT and documentation skills
  • Ability to work independently within a clientโ€™s home
  • Willingness to learn, adapt, and take feedback

Desirable

  • Experience supporting clients in community-based settings
