B6 Specialist MH Team- Duty Worker
Day Webster
The hours are 37.5 hoiurs per week, Monday to Friday, 9am-5pm.
The applicant will be a dedicated duty worker at the Specialist Mental Health team (formerly known as CMHT).
Provide assessments and prioritise appointments.
Provide urgent assessment for individuals who present with acute mental health needs.
Following assessments to be responsible for identifying appropriate management plans, follow up care, referrals. Refer patients for mental health act assessments if required.
Update care documents including risk assessments and care plans, as required.
Provide brief crisis interventions and follow up appointments for individuals to support crisis management for a time limited period. Make referral to the Home Treatment team, as required.
There are a small number of patients who do not yet have allocated care coordinators at present. These cases are currently managed by duty, and the worker will need to provide regular telephone and face to face appointments with these clients.
Take part in MDT working and be part of zoning meetings.
Escalate concerns to consultant psychiatrist and management team.
What you need – Role requirements
- Degree or Diploma in Social Work, OT or RMN
- Active Professional registration with Social Work England (SWE), HCPC or NMC (for RMN's)
