About the Role

Are you passionate about supporting people in your community?

We are looking for a caring, motivated, and compassionate Band 5 Community Worker to join our team across the East of England. You will play a key role in helping individuals maintain their independence, improve their wellbeing, and access the support they need within their own homes and communities.

What You'll Do

• Support service users with their health, wellbeing, and social care needs

• Carry out assessments and contribute to person-centred care plans

• Work closely with healthcare professionals, families, and partner agencies

• Promote independence and positive health outcomes

• Keep accurate and timely records

• Follow safeguarding, confidentiality, and professional standards

• Participate in team meetings, supervision, and training

What We're Looking For

Essential

Minimum 6 months of  NHS or community healthcare experience

Experience working with vulnerable adults, families, children, or community services

Good organisational and time management skills

Understanding of safeguarding and confidentiality

Full UK driving licence and access to a vehicle
