We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing care services, ensuring clients receive personalised support tailored to their individual needs. Duties

  • Coordinate and schedule care plans for clients, ensuring timely and appropriate support.
  • Liaise with healthcare professionals, clients, and their families to facilitate effective communication and service delivery.
  • Maintain accurate and up-to-date records of client information, care schedules, and service documentation.
  • Provide administrative support including managing appointments, preparing reports, and handling correspondence.
  • Monitor client progress and adjust care plans in collaboration with relevant stakeholders.
  • Ensure compliance with organisational policies and relevant health and safety regulations.
  • Assist in the onboarding of new clients and staff members as required.
  • Taking on the responsibility of having the on-call and ensuring all calls are answered and actioned in a professional manner.

Requirements

  • Previous Care Coordinator experience is essential to effectively manage administrative tasks.
  • Excellent organisational skills with the ability to prioritise tasks efficiently.
  • Strong communication skills, both written and verbal, to liaise effectively with clients, families, and healthcare professionals.
  • Compassionate approach with a genuine desire to support individuals in need of care.
  • Ability to work independently whilst also being a team player.
  • Good computer literacy, including familiarity with standard office software programmes.
  • Knowledge of Birdie - Monitoring Software
  • Flexibility to adapt to changing priorities within a dynamic environment.
  • Able to work evening , weekdays and bank holidays
