SW Locums is recruiting an experienced Children’s Social Worker for a full-time locum opportunity within the Transitions and Leaving Care Service at the London Borough of Bexley.

About the Role

You’ll support young people in care and care leavers as they develop independence skills and prepare for adulthood.

Key responsibilities include:

• Completing assessments and personalised pathway plans

• Supporting access to education, employment, accommodation and health services

• Promoting the voice and aspirations of young people

• Working with housing, education, health and community partners

• Undertaking visits, reviews and statutory visits

• Assessing risk and identifying support needs

• Advocating for young people's rights and wellbeing

• Maintaining accurate case records and documentation
