About the role

As our Community Care Manager, you'll lead the delivery of safe, person-centred care by building strong relationships with customers, families and healthcare professionals. You'll oversee customer assessments, care planning and reviews while ensuring every service is compliant, effective and tailored to individual needs.You'll drive quality through audits, spot checks, investigations, safeguarding and continuous improvement, helping us maintain the highest standards of care.
