What you can add:

· Be part of a team and provide a specialist skilled Community Occupational Therapy Service. Working within the Trust policies and guidelines, as an autonomous practitioner, manage a caseload of clients with health needs, often of a complex nature, using evidence based/client centred principles to assess, plan, implement and evaluate interventions.

· Undertake a holistic assessment of clients and carers within their home environment and community setting, including residential care settings.

· Following assessment, interpret and analyse both clinical and non-clinical information in a range of multi-factorial complex acute and chronic conditions in order to identify clients’ needs.

· Following assessment of client and identification of Occupational Therapy goals, implement appropriate treatment plan which may include provision of equipment and minor adaptations.

· Ensure that up to date written and electronic records and activity data are maintained in accordance with Professional and Trust standards.

· Undertake screening and prioritising of referrals to ensure a responsive service.

· Based on Occupational Therapy assessment of client/carers needs, to undertake, initiate and co-ordinate major adaptations, e.g., stair-lifts, bathroom alterations, in conjunction with statutory authorities.

· Liaise with and advise clients, carers and other Health Care Workers and outside agencies on manual handling issues.

· Advise clients, relatives, carers and other professionals on managing issues arising from client’s physical / psychological / cognitive disability within the Occupational Therapist professional expertise.

· Fitting and retrieval of equipment into patients’ homes, e.g. bed raisers, bathroom equipment, which may require working in restricted positions.

What you’ll need:

· Degree/Diploma in Occupational Therapy

· Current Health Professions Council registration

· Current full driving licence - Able to meet the travel requirements of the role.

· A range of post registration experience including in a health and social care setting.

· High level of knowledge relating to disability equipment.

· A good level of knowledge in a wide range of physical conditions.

· Excellent communication skills, written and verbal.

· Registration with the relevant Registration Body if applicable.
