Our client Hackney council is looking for an Occupational Therapist to join their Sensory service.

PURPOSE OF THE JOB

  • Responsible for carrying out comprehensive assessments within the community for people who have either a permanent or a complex and substantial disability. there has to be something about the provision of services that prevents people from needing an ongoing service
  • Responsible for carrying out assessments under relevant legislation such as the Care Act and Housing Grants, Construction and Regeneration Act, to reduce handoffs and ensure that changes in the environment that facilitate independence are reflected in the care provision.
  • Being part of a Multidisciplinary Team, reviewing the appropriateness and effectiveness of the existing provision of care whilst exploring ways how service users can be supported towards greater independence.
  • Responsible for the effective supervision of Occupational Therapy Assistants and Occupational therapy students as required.
  • Carrying out tasks within the OT and Sensory service which will deliver services that will support Adult Social Care ambitions and vision.
  • Work in partnership with service users, carers, and a range of other professionals and stakeholders to support people to maximise their independence.
  • Responsible for the review of support packages, equipment and adaptations.
  • Responsible for keeping accurate and timely case notes on appropriate systems at all times

ACCOUNTABILITIES

Assessment and provision of equipment and recommendations for adaptations.

  • To work within the relevant legislative framework, local, organisational and national priorities, policies and procedures that underpin all practice.
  • To be knowledgeable of the legislation underpinning practice such as The Care Act 2014, Housing and Regeneration Act 2008, Mental Capacity Act and Housing Grants, Construction and Regeneration Act 1996.
  • Be aware of the local political drivers that influence Hackney policy and practice.
  • To work in a multidisciplinary way to support the neighbourhood's model in Hackney, working with health and social care partners and voluntary organisations

INTEGRATION, TEAMS & NEIGHBOURHOOD:

  • Eight neighbourhood areas have been created in City and Hackney Neighbourhoods, formed as much as possible around natural communities based on GP registered lists. These Neighbourhood areas serve populations of between 30,000 to 50,000 residents. The intention is for Neighbourhood areas to be small enough to provide personal care, but large enough to provide a broad range of resilient services.
  • At the core of each Neighbourhood will be a community-centred, integrated team, working across healthcare, social care, public services, community groups and voluntary agencies and the people it serves. In essence, these are multi-disciplinary front-line teams, which will deliver integrated patient-centred services.
  • It is intended that by working together, staff across different disciplines will experience improved communication between services, share knowledge and expertise, and coordinate care planning and delivery.

Monitor and report

  • To ensure that if specialist equipment is provided that it achieves its purpose of improving levels of independence or ease of care safely and effectively.
  • To provide concise, accurate and objective verbal and written reports, paper-based / IT records and correspondence
