We are looking for a dedicated and motivated Social Worker to join the Longer Term Care Team, supporting adults to achieve greater independence and improved quality of life.
This is an excellent opportunity to work within a person-centred, strengths-based service, supporting adults with care and support needs, carers, and vulnerable individuals through assessment, care planning, safeguarding, and long-term intervention.
The Role
As a Social Worker within the Longer Term Care Team, you will manage your own caseload while working collaboratively with citizens, carers, healthcare professionals, and partner organisations to deliver high-quality, outcome-focused support.
Key responsibilities include:
Undertaking assessments in line with statutory duties and relevant legislation.
Developing, implementing, reviewing, and monitoring person-centred care and support plans.
Supporting adults with physical disabilities, mental health needs, illness, or age-related frailty to achieve their personal outcomes.
Working closely with carers, families, statutory services, and voluntary organisations to promote independence.
Managing a varied caseload, prioritising work effectively, and maintaining accurate electronic case records.
Completing safeguarding enquiries and ensuring adults at risk receive appropriate protection.
Preparing high-quality reports and contributing to complex decision-making where required.
Participating in multidisciplinary working and contributing to service improvement.
Supporting students, newly qualified staff, or colleagues through mentoring and supervision where appropriate.
Engaging in continuous professional development, with opportunities to train as an Approved Mental Health Professional (AMHP), Best Interests Assessor (BIA), DOLS Assessor, or other specialist roles.
About You
To be successful in this role, you will have:
A recognised Social Work qualification (Degree, DipSW, CQSW or equivalent).
Current registration with Social Care Wales.
Experience working within Adult Social Care, supporting vulnerable adults in a statutory setting.
A sound understanding of relevant legislation, including the Social Services and Well-being (Wales) Act, Mental Capacity Act, Mental Health Act, safeguarding legislation, and Human Rights principles.
Experience completing assessments, managing complex cases, and working within a strengths-based or recovery-focused approach.
Excellent communication, analytical, and report-writing skills.
The ability to build effective relationships with citizens, carers, and partner agencies.
Strong organisational skills and confidence using electronic case management systems.
