Reablement Care Workers – Community Based

2 Vacancies Available | 21 or 35 Hours per Week | Interviews Being Arranged

Are you a compassionate, reliable and motivated individual who enjoys making a real difference in people's lives?

We are currently recruiting for 2 Reablement Care Workers (Community Carers) to support individuals in their own homes, helping them regain their confidence, independence and everyday living skills following illness, injury or a hospital stay.

This is an excellent opportunity for experienced care workers looking for a rewarding new role within the community.

Unlike traditional domiciliary care, reablement focuses on helping people do things for themselves rather than doing everything for them. You'll play a vital role in supporting individuals to rebuild their abilities, achieve personal goals and live as independently as possible.

The Positions

We have two opportunities available:

  • Position 1: 21 hours per week

  • Position 2: 35 hours per week

  • Shift Pattern: 4-week rolling rota

  • Working Hours: Earlies, lates and potentially long days (6:00am–7:00pm)

  • Weekend Working: Every other weekend

  • Location: Community-based, travelling between service users across the borough

Final rotas and shift arrangements are subject to confirmation.

Your Key Responsibilities

As a Reablement Care Worker, you will:

  • Provide person-centred care and support to individuals in their own homes.

  • Assist with personal care, including washing, dressing and daily routines.

  • Support with preparing meals, drinks and other everyday activities.

  • Assist with medication routines in accordance with individual care plans.

  • Encourage mobility, independence and the development of daily living skills.

  • Help individuals rebuild confidence following illness, injury or hospital discharge.

  • Support service users to achieve agreed goals and maximise their independence.

  • Work closely with occupational therapists, nurses, social workers and other healthcare professionals.

  • Monitor progress and report any changes in an individual's needs or wellbeing.

  • Promote dignity, choice, respect and independence at all times.

What We're Looking For

We're looking for caring, dependable individuals who are passionate about supporting others.

Essential Requirements:

  • Access to a vehicle insured for business use, with the ability to travel between service users across the borough.

  • A compassionate, patient and person-centred approach.

  • Good communication and interpersonal skills.

  • Ability to work independently and as part of a wider care team.

  • Flexibility to work a rolling shift pattern, including every other weekend.

  • A reliable, professional attitude and a commitment to delivering high-quality care.

  • Willingness to attend a face-to-face interview and complete all necessary safer recruitment and pre-employment checks.

Previous experience in care, community support, domiciliary care or reablement would be advantageous.

Why Consider This Opportunity?

  • Two positions available offering either 21 or 35 hours per week.

  • Rewarding, community-based work supporting people in their own homes.

  • The opportunity to make a genuine difference to people's recovery and independence.

  • Varied responsibilities within a person-centred care environment.

  • The chance to work alongside other health and social care professionals.

  • Interviews being arranged, with the potential for a prompt start following approval and successful completion of required checks.
