The Support Worker will provide high-quality care and support to children and young people in line with their individual care plans and the home’s ethos. The role involves creating a safe, nurturing environment where young people feel valued and supported, helping them achieve positive outcomes in education, health, and personal development.
- Care, Support & Safeguarding - Provide consistent, high-quality care and support to young people in line with their individual care plans, risk assessments, and placement plans. - Actively safeguard and promote the welfare of children and young people in accordance with Serene Surroundings’ Safeguarding Policy, Children’s Homes Regulations, and Ofsted standards. - Maintain appropriate professional boundaries at all times and act as a positive role model.
- Observe, record, and report any safeguarding concerns, incidents, or changes in behaviour immediately in line with safeguarding procedures. - Daily Living & Practical Support - Support young people with daily routines, including personal care (where appropriate), meals, education attendance, appointments, and leisure activities. - Encourage independence, life skills development, and age-appropriate decision-making. - Support young people to maintain personal hygiene, tidy living spaces, and develop self-care skills. Emotional & Behavioural Support - Build trusting, respectful relationships with young people, providing emotional support and stability. - Manage challenging behaviour using approved behaviour support strategies, de-escalation techniques, and positive behaviour support plans. - Participate in and support key work sessions, helping young people work towards their individual goals. Education, Health & Well-Being - Promote engagement in education, training, or employment as appropriate to the young person. - Support young people to attend health, mental health, and therapeutic appointments. - Encourage healthy lifestyles, including balanced nutrition, exercise, and emotional well-being. Recording, Reporting & Communication - Maintain accurate, timely, and factual records, including daily logs, incident reports, medication records, and key work notes. - Participate in handovers, team meetings, supervision, and training sessions. - Communicate effectively with colleagues, managers, external professionals, families (where appropriate), and other agencies. Medication & Health and Safety - Administer, record, and store medication safely in line with training and organisational procedures. - Follow all health and safety policies, including fire safety, infection control, and lone working procedures. - Contribute to maintaining a safe, clean, and welcoming home environment.
