Social Work Officer

Good Afternoon,

I am currently representing Haringey Council who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience

We are looking for a Social Work Officer this role will be based at: Hybrid N22 6UX,

The right candidate will:

• Support the discharge planning process for adults requiring social care intervention following hospital admission.
• Undertake non-complex Care Act assessments, reviews, reassessments, and support planning activities under appropriate supervision.
• Gather, record, and analyse information to support Social Workers in completing assessments and care planning.
• Facilitate referrals and coordinate services including domiciliary care, residential and nursing placements, reablement, community support, and voluntary sector provision.
• Support Mental Capacity Act processes, including capacity assessments, best-interest documentation, and information gathering where appropriate.
• Assist with safeguarding enquiries and related activities under the guidance of qualified practitioners.
• Liaise effectively with patients, carers, families, health professionals, community partners, and service providers to facilitate safe and timely discharge arrangements.

We require the following:

• Examples and evidence which demonstrate your genuine motivation for continuing to develop a career in Adult Social care
• Awareness and understanding of the part that Adult Social Care plays in maximising the independence of vulnerable Adults – demonstrably gained experience or exposure to social care in a voluntary, paid or personal basis
